ARAT手臂动作调查测试之深探 - 知乎切换模式写文章登录/注册ARAT手臂动作调查测试之深探贵州三易康复-乐享生活之美!ARAT手臂动作调查测试(Action Research Arm Test)是什么?图1:带测试模板和电动升降OT桌的DynaArm手臂动作调查测试桌ARAT是英文Action Research Arm Test的首字母缩写,中文译名为手臂动作调查测试,也有人翻译为动作研究手臂测试,它是一种国际通行的上肢功能测试方法,用于评估大脑皮层损伤导致偏瘫的个体肢体功能的具体变化。它可以评估受试者处理不同大小、重量和形状物体的能力,因此可以被视为评估手臂特定的活动受限的一种有效方法。ARAT手臂动作调查测试的发展历史1981年,罗纳德.莱尔教授在卡罗尔教授开发的上肢功能测试(Upper Extremity Function Test,UEFT,国内也翻译为卡罗尔上肢功能测试或Carroll上肢功能评定法)的基础上进行了简化,无论是测试项目还是测试时间,都大大地缩短了。基于UEFT基础上开发的ARAT,采用了古特曼量表的方法(Guttman Scale)。虽然进行了改进,但是测量过程中需要更多地明确受试者的体姿、测试细节、评分等等, 约兹巴蒂兰(Yozbatiran)、德尔耶吉亚安(Der-Yeghiaian)和克莱默(Cramer)等学者在2008年提出了全新的标准化ARAT测试概念。ARAT手臂动作调查测试的测试项目ARAT一共19个测试项目,分为四组子量表项目(subscales):抓(Grasp)、握(Grip)、捏(Pinch)以及粗大运动(Gross Movement)。所有的子量表项目都按照古特曼量表的方法进行难度排序,按照这种逻辑,只要受试者能完成该组中难度最大的测试项目,那么她/他肯定能完成难度更小的测试项目,同样道理,若受试者不能完成该组中难度最小的测试项目,那么她/他也肯定不能完成难度更大的测试项目。ARAT手臂动作调查测试的测试逻辑按照莱尔(Lyle)设定的限制规则,受试者首先需进行一个组子项目中难度最大的测试项,若该项得到了最高分3分,则该组其余子项目无需再进行测试,均打3分。比如第一组任务包括6项子任务,若第一项测试“抓放最大号积木”正常完成,计3分,则整个项目直接计分6*3=18分,第2-6项则不用进行测试,直接跳到第7项测试(也就是第二组的第1项)。若该组的第一项测试任务得分为0分、1分或者2分,都要进行该组第二项测试任务。每一组的第二项是该组中难度级别最低的,若该项得分为0分,则整组所有项目都计分为0分(即使前面第1项得分不是0分),第3-6项则不用进行测试,直接跳到第7项测试(也就是第二组的第1项)。若一个组第二项测试为1分、2分或者3分,都要继续进行之后的项目测试。前面我们说过,ARAT一共19个测试项目,分为四组子量表项目,那么,测试最少的项目至少是4项,最多则是19项。ARAT手臂动作调查测试的测试姿态要求及顺序ARAT必须在一个规范的环境下进行测试,因为需要标准化的桌子和座椅。在开始测试之前,患者必须坐在一把带直立靠背且无扶手的座椅上。在整个测试过程中,受试者的背部必须与座椅靠背保持接触,若受试者的背部脱离了座椅,则需要及时进行提醒与纠正。测试过程中,受试者的脚面也必须一直与地面接触。ARAT的测试需要对双手进行测试,首先应当对健肢或受影响较小的上肢进行测试,以便获取受试者的基准分值(Baseline Scores)。为了使受试者充分理解测试的要求和过程,测试人员(医生)应当进行慢速、正确的演示,并让受试者进行一次试测试。对于除了粗大运动外,每一个动作任务的手掌起始位和结束位来说,都要求为旋前位(掌面接触桌面),而粗大运动项目的测试,则要求受试者将手掌摆放为旋后位(掌面朝上)。ARAT手臂动作调查测试的计分ARAT手臂动作调查测试的计分为四级式计分,也就是:0分 - 不能完成测试的任何一部分,或者完成时间超过60秒1分 - 完成部分的测试内容,完成时间不超过60秒(若受试者正在完成动作,但时间已经到60秒,测试者可以叫停,之后计分1分)2分 - 完成测试,但用时达到或超过了5秒,但不超过60秒3分 - 正常完成测试,且用时少于5秒为什么ARAT手臂动作调查测试要实现模板化和标准化?目前,市面上很多ARAT手臂动作测试套件均为木箱式结构,这种ARAT套件最大的特点就是便携箱-所有的配件都容纳进了一个木箱子里面,同时把木箱本身当作高处置物架(ARAT很多测试项目涉及高处置物架!),但是其缺点也是显而易见的:图2:老版本的ARAT测试套件,简单一个箱子,因为是连体式设计,很多动作没法好好开展图3:老式ARAT的包装第一、没有独立式高处置物架。老版本为依托便携箱进行高处置物操作,不能根据患者的手臂长短或痉挛情况调节高处置物架的距离,具有很大的局限性。在ARAT的19项操作中,有12项必须要使用高处置物架进行操作。而且其高处置物的面积较小,不利于受试者将物件摆放在其上面,再加上有锁扣等障碍物,极大影响测试效果。第二、没有配备标准化的电动升降桌和座椅。因为受试者的身高不同,若采用固定高度的OT桌进行测试,而不考虑患者的身高臂长等因素,测试的准确性和测试结果大受影响。若测试座椅为普通座椅(如倾斜式靠背或者带扶手的座椅),更加不符合ARAT的测试规范,不能正确观察受试者是否进行了身体的代偿(后背离开座椅靠背),扶手更有可能会妨碍受试者进行正常测试。第三、配件稳定盒没有内衬。配件稳定盒的作用是在进行带有钢珠或弹子球的操作中为防止钢珠或弹珠滚动提供稳定作用。用于ARAT的第11、12、13、14、15、16项操作。老版本配置了配件盒,但是没有配置内衬,当放入钢珠或滚珠后,钢珠弹珠容易滚动,对于受试者开始进行动作操作产生很大的影响。第四、标准化木球。用于进行ARAT第5项动作操作。按照ARAT的要求,球体的直径为7.5厘米。老版本带有一个直径7.0厘米、7.2厘米等不同规格的美式板球。首先其直径不符合ARAT的操作规范,板球本身为软体材质,经抓握后有可能出现凹陷,无法真正考核受试者抓握7.5厘米直径物体的能力。图4:标准化的量杯,毫升和盎司双刻度显示,测试结果更加准确!第五、操作模板。所有的老版本都没有操作模板,很多医生(护士)对物件的摆放根本无从下手,若无法正确摆放测试物件的位置,其测试结果肯定会出现极大的偏差,若是由不同的测试人员(医生或治疗师)对同一个受试者进行两次测试,由于摆放位置没有规范,测试结果往往会截然不同。图5:操作模板让测试实现了标准化,大大提高了测试的准确性,节省了您宝贵的时间(40%以上!!)结束语DynaArm手臂动作调查测试评估桌带有操作模板、独立式高处置物架、电动升降OT桌以及标准化座椅,解决了ARAT测试中遇到的各种问题,极大地提升了ARAT测试的标准化程度,提高了其可重测性,测试结果更加准确。若您想了解更多关于DynaArm手臂动作调查测试评估桌以及ARAT测试的物品准备、起始位、任务描述、计时过程、口头引导语、关键动作要素分析、评分细节以及测试注意事项,以及其它诸如模板化(Jebsen)杰布森手功能评估箱、沃尔夫(Wolf)运动功能测试量表桌(WMFT)、卡罗尔(Carroll)上肢功能测试套件、七级偏瘫上肢功能测试评估套件(FTHUE-HK)、格拉斯普上肢量表评估箱(GRASSP)、范李肖特四肢瘫痪手功能测试套件(VLT)、日本金子翼原版简易上肢机能检查箱(STEF)以及上肢机能检查箱(MFT)等上肢综合功能测试产品的更多信息,请与贵州三易医疗器械有限公司联系,并随时关注我们的知乎官方账号。发布于 2021-06-24 17:18康复治疗康复医学物理疗法与作业疗法概论(第二版)(书籍)​赞同 1​​添加评论​分享​喜欢​收藏​申请

上肢动作研究量表(Action ResearchArm Test,ARAT)是一种国际通行的上肢功能测试方法,用以评估大脑皮层损伤导致偏瘫的个体肢体功能的具体变化。它可以评估受试者处理不同大小、重量和形状物体的能力,因此可以被视为评估手臂特定的活动受限的一种有效方法。

ARAT基于Caroll上肢功能测试(Upper Extremity Function Test,UEFT)发展而来,一共19个测试项目,分为四组子量表项目,包括4个基本动作: 抓、握、捏以及粗大运动。每项为4级评分(0~3分),0分表示无法完成动作,3分表示可正常完成动作;一侧上肢的评分范围为0~57分,分数越高,表示功能越好。每个受试者所需的评测时间约10min左右。

评分标准

0分:不能进行任何动作,或者完成时间超过60s。

1分:能够完成部分测试内容,完成时间不超过60s。

2分:能够完成,但完成困难或有时间延迟,包括存在异常的手部、上肢运动成分、身体姿势异常、用时超过5s但不超过60s。

3分:能正常完成,各个动作正常,用时小于5s。

如果受试者有手指义肢,除了捏测试,其余正常进行。任何需要截肢部分完成的动作测试,得分计为0并标注“因为截肢无法进行”。

测试逻辑

如果患者在第一项(最难一项)得分为3分,则该子测试中所有项目为满分,然后直接转到下一个子测试。

如果第一项低于3分,则进行第二项任务(最简单一项)。

如果第二项得分为0,则该子测试总分为0,第3-6项无需进行,转到下一个子测试继续。

如果第二项得分高于0分,剩余项目都要进行。

测试准备

将ARAT手臂动作调查测试评估箱放在桌子上,距离桌缘15cm,患者坐在没有扶手的凳子上,双臂 置于桌子上。

治疗师解释测试要求并在必要时演示动作,让患者模仿。

先从患者健侧或轻症患侧开始,然后从患肢开始。

测试任务之间允许中断,部分任务之间不可以中断。

治疗师不能给予帮助。患者不能多次练习。

子测试一:抓取和放置

患者垂直抓取测试物件,并将其放置在木箱顶部 1. 抓取并放置10cm宽的木块。如果此项得分为3分,则子测试总分为18分,转到下一个子测试;低于3分,进行下一项。 2. 抓取并放置2.5cm宽的木块。如果此次项得分为0分,则子测试总分为0分,转到下一个子测试。 3. 抓取并放置5cm宽的木块。 4. 抓取并放置7.5cm宽的木块。 5. 抓取并放置木球。 6. 抓取并放置石块。

子测试二:抓握和放置

抓握起测试物件,并将其放置在指定位置,测试动作完成必须包括前臂旋前 1. 将水从一个杯子倒到另一个杯子。如果此项得分为3分,则子测试总分为12分。 2. 放置2.25cm金属管到指定位置。如果此项得分为0分,则子测试总分为0分。 3. 放置1cm金属管至指定位置。 4. 放置垫圈至指定位置。

子测试三:捏取和放置

手指对捏测试物品,并放到指定位置 1. 用拇指和无名指,捏取6mm钢珠。如果此项得分为3分,则子测试总分为18分。 2. 用拇指和食指,捏取1.5cm玻璃珠。如果此次项得分为0分,则子测试总分为0分。 3. 用拇指和中指,捏取6mm钢珠。 4. 用拇指和食指,捏取6mm钢珠。 5. 用拇指和无名指,捏取1.5cm玻璃珠。 6. 用拇指和中指,捏取1.5cm钢珠。

子测试四:粗大运动评估

不使用桌子。起始患者坐在椅子上,双臂放在大腿上 1. 用手摸后脑勺。如果此项得分为3分,则子测试总分为9分。能够抬起手臂记为1分。 2. 用手摸头顶。如果此次项得分为0分,则子测试总分为0分。 3. 用手摸嘴。

临床上脑卒中上肢运动功能评估常用量表有Fugl-Meyer上肢运动功能评定量表(Fugl-Meyer Assessment Upper Extremity Scale,FMA-UE)、上肢动作研究量表(Action Research Arm Test,ARAT)等。

FMA是一种设计良好,比较全面,同时对脑卒中后功能障碍较为有效的临床评估手段。Twitchell和Brunnstrom在对大量脑卒中患者的恢复过程进行观察中先后发现,此为一系列过程,并提出了脑卒中偏瘫运动恢复六阶段理论,瑞典学者Fugl-Meyer主要根据上述观点,依据脑卒中偏瘫患者运动功能恢复的Brunnstrom六阶段分期研发出FMA量表,其评估的内容主要包括肢体运动(包括上肢和下肢)、平衡、感觉、关节活动度和疼痛这五项。其中运动功能模块共100分(上肢66分、下肢34分)。FMA-UE共包括33个上肢近端及远端肢体运动相关条目,每项按0、1、2计分,0分为完全不能完成,2分为可以完成规定动作,1分介于二者之间;总分数在0-66分不等。FMA-UE主要是评估偏瘫侧上肢的反射活动、运动控制和肌肉力量,它被广泛用作康复试验和记录脑卒中后恢复的结果指标。FMA主要的缺陷是对轻度损伤的患者有天花板效应。

ARAT是一个标准的测量上肢及手功能的量表。ARAT从4种基础动作解释并评估日常生活中上肢复杂动作,包括肩肘屈伸下的抓、握、捏、粗大运动。这个测试是评估受试者能否拿起不同尺寸的物体到37.5cm高、圆柱形物体37.5cm远、拇指分别同示指、中指等捏起不同尺寸的物体,最后进行3项上肢粗大运动。每侧上肢独立接受评估,ARAT是0、1、2、3式计分量表(一侧肢体总分是57分),3分代表任务得到很好的完成;2分指的是任务虽然完成,但是过程较为困难,或者协调性不好,过程时间延长了;1分代表的是动作只有部分完成;0分代表的是动作完全不能完成。该量表可用于脑卒中患者的评估,但是它的局限主要体现在:1.没有对动作的灵巧性进行测试;2.需要一套标准化评估工具;3.采用0,1,2,3半定量式计分;4.具有天花板和地板效应(下附ARAT量表及指导)。

Action Research Arm Test (ARAT) Kit The Action Research Arm Test (ARAT) is an evaluative measure to assess specific changes in limb function among individuals who sustained cortical damage resulting in hemiplegia (Lyle, 1981). It assesses a client’s ability to handle objects differing in size, weight and shape and therefore can be considered to be an arm-specific measure of activity limitation (Platz, Pinkowski, Kim, di Bella, & Johnson, 2005). The ARATs is a 19 item measure divided into 4 sub-tests (grasp, grip, pinch, and gross arm movement). 上肢动作研究试验(ARAT)(Action Research Arm Test (ARAT))是一个用来评估由于大脑皮层受到持续损伤而导致肢体功能瘫痪的个体上肢功能具体变化的指标(Lyle,1981)。它通过让客户操作不同大小、重量和形状的物体来评价其能力,因此被认为是一种特别测试手臂活动限度的方式(Platz, Pinkowski, Kim, di Bella, & Johnson, 2005)。

经验证,与其他量表相比,上肢动作研究量表(ARAT)在脑卒中患者中具有良好的会聚效度、区分效度和同时效度。

ARAT中的19项评估被分成4个子测试(抓取和放置GRASP,握取和放置GRIP,捏取和放置PINCH,手臂简单粗大运动GROSS MOVEMENT)。每一项目的表现通过以下4个等级进行得分评价:

3分: 测试完成得很完美。

2分: 完成了测试。但明显比未受影响的一侧需要更多的时间和/或有较大的困难。动作表现的慢和笨拙。

ARAT手臂动作调查测试评估箱-上肢动作研究量表(ARAT)套件 

 

ARAT上肢动作研究量表(Action Research Arm Test),是一个用来评估由于大脑皮层受到持续损伤而导致肢体功能瘫痪的个体上肢功能具体变化的指标(Lyle,1981)。它通过让客户操作不同大小、重量和形状的物体来评价其能力,因此被认为是一种特别测试手臂活动限度的方式(Platz, Pinkowski, Kim, di Bella, & Johnson, 2005)。

经验证,与其他量表相比,上肢动作研究量表(ARAT)在脑卒中患者中具有良好的会聚效度、区分效度和同时效度。

ARAT中的19项评估被分成4个子测试(抓取和放置GRASP,握取和放置GRIP,捏取和放置PINCH,手臂简单粗大运动GROSS MOVEMENT)。每一项目的表现通过以下4个等级进行得分评价:

3分: 测试完成得很完美。

2分: 完成了测试。但明显比未受影响的一侧需要更多的时间和/或有较大的困难。动作表现的慢和笨拙。

1分: 完成部分测试。能够从工作平台上拿起或抬起测试物件,但无法将其放置在正确的位置。比如,在“将水从一个杯子倒到另一个杯子”的测试项中,能够拿起杯子,但无法将水倒进另一个杯子中。在第4项评估大项(简单粗大运动评估)中,能够抬起手臂,得分为1。

0分: 不能完成测试任务的任何部分。

 

如果子测试的第1项(最难的项目)得分为3分,患者将获得该子测试所有项目的满分。然后转到下一个子测试继续测试。

如果子测试第一项得分低于3分,则使用该子测试第二项(每个子测试中最简单的一项)。

如果子测试第二项得分为0分,则该子测试总得分为0分,应该转到下一个子测试继续测试。

Action Research Arm Test

Welcome to the Action Research Arm Test (ARAT) Training and Assessment Programme. The ARAT is one of the most commonly used scales to assess upper limb function after a stroke. The aim of this website is to introduce the ARAT, provide a description of the scale and guide researchers and clinicians into how to administer the test.

ARAT training programme (currently under development): This will introduce the ARAT and provide an online training programme for administering the ARAT. You can learn how to administer and score ARAT by completing the ' '

Assessment programme: It is important for the validity of the test that all raters (researchers and clinicians administering the ARAT) conducting the ARAT and score the test in the same way. The assessment programme on this website allows ARAT raters to score videos of participants completing the ARAT to ensure correct administration of the test. These phases are completed in two parts to test scores between raters as well as the score of the same rater over time.

If you have already completed Assessment Programme Phase 1 of the assessment and you are required to undergo a one month assessment then you need to complete 'Assessment Programme Phase 2'.

ARAT手臂动作调查测试评估箱-上肢动作研究量表(ARAT)套件

ARAT中的19项评估被分成4个子测试(抓取和放置GRASP,握取和放置GRIP,捏取和放置PINCH,手臂简单粗大运动GROSS MOVEMENT)。每一项目的表现通过以下4个等级进行得分评价:

3分: 测试完成得很完美。

2分: 完成了测试。但明显比未受影响的一侧需要更多的时间和/或有较大的困难。动作表现的慢和笨拙。

1分: 完成部分测试。能够从工作平台上拿起或抬起测试物件,但无法将其放置在正确的位置。比如,在“将水从一个杯子倒到另一个杯子”的测试项中,能够拿起杯子,但无法将水倒进另一个杯子中。在第4项评估大项(简单粗大运动评估)中,能够抬起手臂,得分为1。

0分: 不能完成测试任务的任何部分。
